NOTARY DATABASE INQUIRY - CHANGE INSTRUCTIONS  (rev. 7-7-05)

Status: "Suspended" means that this office does not have either your current notarial bond or E&O policy on file or you have not filed your annual report. Your status will remain suspended and you will have no authority to function as a notary until you comply with notary law.

To file bond: Please provide this office with an original or certified true copy of your current bond signed by the Clerk of Court in your parish and a check or money order for the $20.00 filing fee. Mail it to Secretary of State, Notary Division, P. O. Box 94125, Baton Rouge, LA 70804-9125.

To file Errors and Omissions Policy: Please provide this office with an original E&O Policy and a check or money order for the $20.00 filing fee. Mail it to Secretary of State, Notary Division, P.O. Box 94125, Baton Rouge, LA 70804-9125.

To file annual report: Please contact this office by email at notaries@sos.louisiana.gov , mail, fax (225-922-0945) or phone (225-922-0507), give your current address and indicate you are suspended and need to file annual report. This office will mail the report you need to complete and return with appropriate fee.

Name: If your name has changed, please follow the instructions on the Notary Filing Requirements Chart for NAME CHANGE.
